Provide at least two business names on this form, your first choice and second choice in case your first is not available. You may provide an optional third choice. This is the address where you intend to do business and will usually be included in the filings to the state. Residential homes and apartments are acceptable in this section as well. Be aware that states may not allow virtual or mailstop addresses. If you are doing business in one state and incorporating in another, this would be the address of where the actual business is located, not the state of incorporation.

All states require that your LLC maintain a registered agent. A Registered Agent is simply a person or company responsible for receiving important legal and tax documents on behalf of your corporation.
